I’d like to talk about the southeast side of San Jose for a minute, because families in Evergreen, Silver Creek and the surrounding foothill neighborhoods often assume the good senior living options are all over on the west side. Oakmont of Silver Creek is a big reason that isn’t true. It’s an assisted living and memory care community run by Oakmont Senior Living, and it’s built around a hospitality model where dining, service and daily activities get as much attention as the care itself.
In the tour video above I take you through the property, so you can get a feel for the common areas and grounds before you ever schedule a visit. What I want to do here is fill in the parts a video can’t cover: what daily life looks like, how the care is set up, what questions to ask about cost, and how a longtime east side homeowner might think about selling the house to make a move like this work.
Where it sits and why that matters
Oakmont of Silver Creek is at 3544 San Felipe Road, in the 95135 ZIP code on the southeast edge of San Jose near the Silver Creek Valley and Evergreen neighborhoods. This is a residential part of town with established streets, newer subdivisions and the hills right behind it, so it has a calmer, more suburban pace than the busy commercial corridors elsewhere in the city. Shopping, parks and medical offices are within a reasonable drive, and the community’s own outdoor areas (walking paths, resident gardens, patios and a pet park) mean residents can get fresh air without leaving the property.
For a family that has lived in Evergreen or Silver Creek for 25 years, the location may be the deciding factor. Mom or Dad stays close to the neighbors, the friends and the grocery store they know, and the kids who grew up in the neighborhood (and may still live nearby) can visit without crossing the whole valley at rush hour. The building itself, from the entrance, is a two-story craftsman-style design with a covered porte-cochère and a lot of landscaping out front, which reads more like a large residence than an institution.
What a day at Oakmont looks like
Oakmont’s whole pitch is that residents shouldn’t spend their days on chores, and the service list backs that up. Housekeeping, personal laundry and concierge-style services are part of the package, with extras like room service, pet care and personal fitness training available if a resident wants them. Transportation and specific inclusions vary, so get the current list in writing when you request pricing.
Dining is where Oakmont puts a lot of its energy. Meals are served restaurant-style in the main dining room, with indoor and outdoor seating, and there’s a more casual bistro plus a bar and lounge for residents who’d rather grab something lighter or have a drink before dinner. A private dining room gives families a place to celebrate a birthday or holiday together, which sounds like a small thing until you realize how much it helps a parent feel like they’re still hosting.
Beyond meals, the community lists a movie theater, fitness center, activity room, library, and a salon and day spa. The activity calendar has historically included movie nights, art classes, gardening, live music, outings to restaurants and local attractions, spiritual services, educational talks and volunteer opportunities, along with themed social events and holiday parties. Oakmont has also described fitness classes like strength training, yoga and balance work, which matter a great deal for fall prevention and for keeping residents independent longer.
Care levels at Oakmont of Silver Creek
The community offers two levels of care, and it helps to know how they differ before your first tour. Many families start by looking at assisted living for a parent who’s slowing down, only to realize memory care may be part of the picture within a few years. Having both on one campus means a resident can make that transition without starting over somewhere unfamiliar.
Assisted living
Assisted living at Oakmont is designed for residents who need help with daily activities such as medication management, grooming and mobility. Trained caregivers are available around the clock, and care plans are built for each resident and revisited through regular wellness assessments as needs change. Oakmont has also described having a nurse on staff, health screenings, fall prevention programming and coordination with outside doctors, and it has mentioned a wellness program for residents managing diabetes. Ask about current nurse staffing hours and how the care plan review process works, since those details can change over time.
Memory care
For residents living with Alzheimer’s or another form of dementia, Oakmont offers memory care in a secured area of the community with private and companion suites. The programming centers on structured activities, music, art and other engagement designed to create meaningful moments during the day. Memory care decisions are some of the hardest a family makes, and it’s worth doing some reading before you compare communities; the Alzheimer’s Association has solid guides on stages of the disease and what to expect from care settings.
Whichever level of care you’re considering, check the community’s state record. Assisted living and memory care in California are licensed as Residential Care Facilities for the Elderly, and you can look up Oakmont of Silver Creek’s license, capacity and inspection reports on the California Department of Social Services Care Facility Search. I recommend every family do this before signing, for any community they’re considering.
Apartments and what they’ll cost
According to Oakmont of Silver Creek’s website, apartment homes range from studio suites up to two-bedroom residences, and Oakmont describes them as among the larger units in the industry. Earlier descriptions of the community mentioned features like walk-in showers, and in some plans balconies and full kitchens, so if any of those are important to you, ask which specific floor plans include them. Companion suites are also an option, which can bring the cost down for residents open to sharing.
Oakmont doesn’t publish rates on its website as of September 2026, and it positions itself at the upper end of the San Jose market. That means you’ll need to call for a quote, and when you do, I’d push for detail on a few things: what the base monthly rate covers, how care costs are calculated and assessed, how memory care pricing is structured, and what the annual increases have looked like the last few years. Private pay is the norm here, so the family’s own assets (usually the house) are going to be doing most of the work.
Selling the east side family home to make the move
This is the part of the conversation I have most often, and it’s where my world as a REALTOR® overlaps with senior living. Oakmont is a monthly rental with no large buy-in, so you aren’t trying to come up with a big entrance fee the way you would at a continuing care retirement community. Instead, the goal is to turn the equity in the home into a dependable source of funds that covers the monthly cost for years, and in a neighborhood like Evergreen or Silver Creek that equity is often substantial.
The first decision is sequencing, and there’s no one right answer. If there’s enough cash or a bridge loan available, moving your parent in first and then selling the empty house is usually smoother, because nobody has to live through showings and the home can be prepped properly. If the proceeds are needed up front, we plan the sale and the move date together so there’s no gap. Compass Concierge can cover the up-front cost of prep like paint, flooring and staging, repaid at closing, and some homes are better sold as-is so the family doesn’t sink time into repairs.
There’s also the tax side to consider. A longtime owner who’s used the house as a primary residence for two of the last five years may be able to exclude up to $250,000 of gain, or $500,000 for a married couple, under IRS Section 121, and a CPA should confirm how that applies in your case. If a healthy spouse or the family plans to buy a smaller home rather than having both parents move to Oakmont, California’s Prop 19 may let a homeowner 55 or older carry their existing property tax base to the replacement home, though it doesn’t apply to a rental like Oakmont. Then comes the hardest part, which is sorting through decades of belongings, and I’d give that its own schedule rather than squeezing it into the last two weeks. If you’d like to see how this might work for your family, take a look at how I approach it on my selling with Seb page.
Keeping the family in the loop
Oakmont treats the move as a family decision rather than just an admission, and it says so in how it handles communication. Families are encouraged to join care planning meetings, come to community events and stay in touch with the leadership team, and regular updates and care conferences are part of the routine. Pets are welcome too, which the on-site pet park confirms, and for a lot of older adults that’s the single detail that makes a move feel possible.
Oakmont has also described intergenerational events that bring in local kids and families. When you tour, ask to see the current month’s calendar and sit in on an activity if you can, because a printed schedule and what actually happens on a Tuesday afternoon aren’t always the same thing, and you’ll learn a lot from watching how staff and residents interact.
